Commander Keen for me. Sure, the games have zero scary visuals and all looks happy and all, but since you die by one hit and are mostly fighting small snails or lame robots, everytime you run into a giant robot that immediatly shoots 20 laser bolts at you as soon as it's in view makes me jump.
I also still hate those face huggers in Duke Nukem 3d, make me jump all the time too. Everytime I hear that damn sound I immediatly start blasting my bazookas at every wall I see.

I used to rush through levels/finish the game so easily back then.. Now it seems super hardcore :/ A heck of a lot more difficult than any recent release anyway!

I found the redead in Ocarina of time scary. Pyramid head was quite the freak. His cameo in the movie was probably the best bit : B

I've got to mention... 3 that scared me a lot.
Alone in the Dark 4 : The New Nightmare (ripped off Resident Evil like hell - thankfully I didn't know that during the time it came out for the first time...)
Then, there are The Blackstone Chronicles... A game based on a serialised novel, some say it's crap, I still think if you'd play it in a darkened room, with full volume and effects/music on, it'd turn out pretty spooky...
Next one... Sanitarium an adventure game in RPG-graphics, interesting and well-written. You wake up in an asylum, your body in bandages, and you have no idea who you are or what you are doing at this place. While playing the game you don't know what's real and what's phantasy anymore, are you really mad and imaginating things, or did something horrible happen for real ? If you find this game, get it, and try to make it run on your machine, 'cause it's really worth the effort.
